The System operates from the MICROSOFT WINDOWS NT or 2000 "platforms" and supports anywhere from 1 to 100+ Work Stations at one or more locations. Practice Management System The Patient Scheduling module within Simplicity+ is unique! A calendar is displayed at all times and the user works in a "what-you-see-is-what-you get" setting. Appointments are "cut-and-pasted" at will and an appointment can easily be moved from 1 day to another or from 1 provider to another as needs dictate. From the Scheduler, all NOTES can be reviewed and the user can easily review - -Balances due -Primary and Secondary Insurance coverages -Copays due -Referring Doctor Information -Authorization status (authorized versus used visits) MORE... Patient Scheduling In addition, in a multi-provider setting, the user can display up to 11 scheduled resources at the same time - and, can schedule appointments easily from that multi-column display. Of equal importance, the user can easily "Edit a day" if the Doctor's schedule changes, "Force" appontments before and after normal hours, and "Multi- book" a time slot when necessary. See Screen View #5 (continued...) PREVIOUS... Debt Management Managing Accounts Receivable within a Practice is oftentimes an issue. The cry heard is "...we don't have enough time..." Simplicity+ addresses this dilemma and provides special, easy-to-use, "tools" that when used properly, do away with the "... I don't have enough time..." myth. Managing Accounts Receivables is done easily and can be handled from 2 on-screen "launching pads". The first deals with delinquency at the Patient level and the second at the Insurance level. Delinquency Control Claims Management On the Insurance side, our CLAIMS MANAGEMENT sub-system allows the user to review the status of all outstanding or "unpaid" claims at will. For example, the user can specify they want to see all claims that are 65 days or older that have either not been refiled or paid. This routine can be run on a carrier-by-carrier basis or in bulk! A result of the "process" is an on-screen display of all accounts that meet the selection criteria. From this "launching pad", the user can review the claims status (where the claim is in the overall payment process), review the refiling history and actually review the procedures entered from day 1! Again, from the screen, action can be taken to refile the claim, change the claim from a Primary to a Secondary, or write-off small balance, etc. The patient level control system allows the user to control the amount of work served up in an on-screen setting. For example, if the user wants to see all the Patients that owed more than $150 that had paid NOTHING to the Practice (except copays) over the previous 90 days, those variables would be key-entered and the screen would display those Patient accounts with Patient level balances (net Insurance) that met the criteria. From the on-screen display, the user can then review any and all NOTES associated with the Account, review the total delinquency on a fee ticket by fee ticket basis, generate a special collection letters or turn the account over for third party collection handling - without leaving the Delinquency Control sub-system. The NOTES sub-systems within Simplicity+ are among its strongest features! Anywhere from within the system a NOTE can be written at the Responsible Party/Guarantor level, or Patient level. A brief statement summarizing the conversation, be it with an insurance company or a patient, is entered. Then the system asks if there is need to follow-up on the issue at some point in the future. If follow-up is in order, appropriate criteria is entered. Then on the follow-up date, "mail" will appear in the user's mailbox! It includes the follow-up notice along with the original note. On the error reporting side, any data- related error detected by GATEWAY or the Insurance Company is displayed on-screen in a launching pad setting that allows the user to correct the error and resubmit the claim without having to "jump" from screen-to- screen. With this capability in place, the need to review printed reports in order to "find- and-fix" claims errors is eliminated! Claims Errors Automatic ELECTRONIC CLAIMS ERROR HANDLING and related CLAIMS STATUS reporting - These sub-systems take electronic information provided by GATEWAY EDI (a MICRO SYSTEMS-ST. LOUIS business partner) and integrates it with each claim filed electronically. On the Claims Status Side, the system, at a fee ticket level, presents an ongoing dialogue that lets the user know the "status" (in terms of payment progress) of each and every claim! For example, a scenario like the following is commonplace . 9/02/2001Accepted by Gateway 9/03/2001Claim Forwarded to United Health Care 9/05/2001Accepted by Carrier 9/14/2001Paid according to plan provisions Claim Status See Screen Views 6 for Claim Errors See Screen Views 7 for Claim Status Currently, only Medicare participates by providing electronic payment remittance. However, the new HIPAA mandate indicates that ALL carriers must begin submitting payments electronically in 2002+. Therefore, today, remittances are received daily and held in inventory. Then, when a EOB arrives - along with the actual check - the standard "payment" routine is activated and the check number is entered into the system, and automatic payment posting begins. The keystrokes that would normally be entered to post a payment are automatically entered and the system knows what, if any, secondary has been "crossed over", or needs to be printed, etc. This process leaves the user with the need to enter a single keystrokes to confirm that the payment has been handled correctly... NOTE: The user can interrupt the process, leave the sub-system, do something else and return to the Payment Posting process and the system would have "book marked" the last payment posted and begins the process with the next payment to be entered! (cont...) Patient Registration Pre-Authorization Procedure Reimbursement Tracking Simplicity+ supports an integrated PRE- AUTHORIZATION capability. When the pre- auth is received, from either the PCP or the Patient, the information is keyed into the system and from that point forward, the system keeps track of authorized versus used visits and this"window" automatically displays when a SB CHARGE is entered or an appointment is scheduled. The sytem also provides a "look ahead" tool that lets the user know in advance of expired patient pre-auths or authorized visits. Procedure Reimbursement Tracking is a unique feature of Simplicity+ . Experience tells us that relatively few procedures contribute significantly to the overall charge/income stream of the Practice. Oftentimes, as few as 15-20 Procedures can account for 80% of the Practice income! As a result, we have built a capability that allows the user to "track" reimbursement on only those Procedures that have a major impact on income! Therefore, if a procedure is "tracked", after the payment is applied to the fee slip in total, a window pops-up allowing for key entry of the amount paid for the specific tracked procedure. The tracked information is then fed into a Management Report that allows the Practice to review, on a periodic basis, reimbursement from different Carriers for the same Procedure. We provide ammunition for the Practice to deal more effectively with Insurance Carriers when they are inconsistent in paying for the very same Procedure! If used properly, it is a very effective "enhancement to reimbursements" tool... A PATIENT REGISTRATION form is automatically generated when a fee ticket (superbill) is printed - if the Patient has not been seen by the Practice in the previous 12 months. The Registration Form contains all pertinent demographics about the Patient who is then asked to review the information "on file" and make corrections where appropriate. Then, while the Patient is seen, staff updates the demographics and a "clean" Registration form is printed and signed by the Patient at the point of departure. The updated form is then attached to the Medical Record jacket and signatures on file issues become mute for another year... SmithKline/Quest Word/WordPerfect Hospital Systems Electronic Medical Records Simplicity+ integrates with these lab order entry/reporting systems. The lab system sits on the "network" and the PATIENT ID is entered into the QUEST system and correct Patient, and ALL related demographics, are extracted from the system and displayed on the QUEST order entry screen. This process eliminates the need to build and maintain two different Patient data files and the Practice is always assured of sending current Patient data to the lab(s). Our staff has built a very unique interface between BOTH of these well known word processing systems. In Simplicity+ , when sitting in Guarantor or Patient record, 2 keystrokes take you to a "bridge" that contains templates of repetitive written tasks. By selecting the needed template - PROGRESS NOTES, REFERRING PHYSICIAN LETTER, a NEW PATIENT letter, etc., the system then jumps to the appropriate word processing system and ALL known information about the Patient is in- place and the user need only key-enter the variable information from the dictation effort... When and where Hospitals will allow Practices to "tie-into" their systems, our staff is ready to DO WHAT IS NECESSARY to make this happen. This could involve simply using the Simplicity+ work station in a dual mode that allows the user to "hot-key" into the Hospital system - OR, it could involve some special programing to find and transfer lab test results, etc. to the database Again, our staff is ready and willing to DO WHAT IS NECESSARY to "tie" Simplicity+ into virtually any EMR system. We offer 2 basic approaches - the "batch" update and the "real time" update and we will use whichever best suits the needs of the Practice. Billing & Collections Today, many Practices prefer to focus on Patient care and "out source" the critical billing/collection tasks. To meet this need we established our MEDICAL BILLING SERVICES GROUP in 1993. This has been a rewarding experience! Now, with significant Billing/Collection experience behind us, we have a much, much better understanding of the needs of the Medical Practice and have "fine tuned" Simplicity+ and our services accordingly. Services offered vary considerably dependent upon the needs of the Practice. For some Physicians, we will "do everything" which means fee tickets, face sheets and authorizations (if required) are mailed, faxed or couriered to our office and our staff key enters the information. In other situations, the using Practice will have 1 or more WINDOWS NT Work Stations installed along with a laser printer and these units would be connected to our Service Center via either direct digital (ISDN) phone lines or our Virtual Private Network (VPN) and the Internet. The Practice-based Work Stations are used to: ·Schedule Patient appointments, ·Key-enter new Patient demographics (and related changes) and ·Key enter all charges... Regardless of which approach outlined above is used, once the Patient and Charge information is entered into Simplicity+ (yes, we use the very same software system that is sold, installed and supported in many of our client offices), our staff goes to work... We file all claims (electronically when possible), post payments, file secondary claims and send statements when balances are due from Patients that are net of Insurance. We also "manage" the Accounts Receivable dollars due the Practice by using many of the "collection tools" that are unique to Simplicity+. These include Patient-based Delinquency Control capabilities that allow us to easily send collection letters and/or make telephone calls as necessary to ensure that Patient balances are paid in a timely manner. On the Insurance side, our CLAIMS MANAGEMENT tools allow us to review, on a demand basis, claims that have not been paid in a timely manner. From within this "system", a claim can be refiled, have its status changed from Primary to Secondary or "leave it alone" as the established payment cycle is in process. In addition to the above, our staff handles all calls regarding account balances from Patients and Insurances alike. Our services are paid for by billing users a small percentage of the dollars collected/entered on behalf of the Practice. In other words, WE DO NOT GET PAID UNTIL THE PRACTICE IS PAID and this is a very strong incentive for doing well on an ongoing basis. The success of our Billing Services operation has exceeded our expectations. Why? It comes down to several key factors. These include - -Our staff is CONSISTENT & PERSISTENT on a day-in-day-out basis, -Our services are priced competitively, -We utilize our own application software system - Simplicity+ - that is feature rich and designed to meet the needs of the busy Physician office and YES, it is the very same software system that currently supports hundreds and hundreds of Physicians in the Metropolitan St. Louis area and, -Lastly, our COMMITMENT TO EXCELLENCE ranks us 2nd to no one in the Billing Services arena. System Requirements The Simplicity+ system operates on the WINDOWS NT/2000 platform on PENTIUM III-IV class computer systems. A system server can be either dedicated or non-dedicated depending upon the number of Work Stations to be connected. Generally, up to 8 Work Stations can be connected to a non-dedicated server with excellent performance! Each Work Station is equipped with a minimum of 64 megabytes of main memory, and at least a 10 gigabyte hard disk drive, CD-ROM drive, 15-17-19" color monitor plus a keyboard and mouse. The system server, be it dedicated or non-dedicated, requires 128-256 megabytes of main memory and at least a 20 gigabyte, 7200 RPM hard disk drive. The remaining components remain the same. We require a semi-dedicated dial-up phone connection (generally shared with the fax machine) for handling electronic claims transmission as well as computer-to-computer sessions between the Practice and our office. Anti-virus tools are a must. If high speed Internet access (DSL or Cable) is intended, a hardware- based "firewall" is required! We prefer to operate in a complete "turn-key" setting that calls for us to provide "everything" associated with the operation. This allows the Practice to count on timely "one-stop- shopping" 100% of the time. Customer Support New Client Orientation Help Desk Staff Training Programming Hardware Total Support Agreements To us, Customer Service is THE key to our success! Granted our system is outstanding and computer systems today are reliable and cost effective but SERVICE "after the sale" is what makes the difference... In fact, we consider ou rselves second t o no one when it comes to supporting our Practices. Uncommon as it may seem, we make ourselves available on a 7/24 basis and a staff person is "on call" 100% of the time... Customer Support is broken down into several key areas. NEW CLIENT ORIENTATION - When a Simplicity+ system is sold Practice orientation begins. In this process, we determine "what is needed" to install the system properly and can include writing custom computer programs to convert existing information to the new setting. STAFF TRAINING - All initial staff training is included in the cost of Simplicity+ and is unlimited. Training is done on-site, on a scheduled basis, and we prefer to work in 1½-2 hour increments with "some" real world interference so the new system is not learned in a sterile vacuum-like setting. The HELP DESK - After the system is installed and staff trained, our support staff is available on an as-needed basis. Telephone calls are always both welcomed and encouraged, and computer-to-computer sessions take place as necessary to either repair the inevitable "bug", or to "shadow" a user to see first-hand how the system is being used. Lastly, we make "house calls" whenever they are deemed necessary. A full time PROGRAMMING staff is on hand to create solutions to new needs, build "custom solutions" that may be unique to a given Practice and handle information conversions when Simplicity+ replaces an existing system. In the HARDWARE arena - Full-time staff both installs and maintains computer systems, operating systems and related communications software. They, too, are available on a 7/24 basis. The staff is both cell phone and beeper equipped so response time is NOW! All software systems (including Simplicity+ ) and computer hardware networking and communications software purchased from MICRO SYSTEMS - St. Louis is warranted for the first full-year of use. In building the new system we have taken great care to retain the ease-of-use and overall simplicity of our current system - because it works! Current users will find that use of keystrokes and function keys will produce the very same result in the true WINDOWS setting. However, we have also added many, many WINDOWS features that will allow current users of WINDOWS based systems to be very comfortable in the setting. For example, an option will allow the user to use the traditional ENTER key to move from field to field OR for the WINDOWS user, the TAB key can be used for the same purpose.
